SKITZO PRODUCTIONS • LIMITED PROMOTIONAL RELEASE

1. Permission and covered session
I give Kelvin Eugene Jordan, doing business as Skitzo Productions, permission to use my image and likeness in photographs and video recorded by Skitzo Productions during the session identified in this form. For video, this includes my recorded voice. If signing for a minor, this permission applies only to the minor identified below. Each other adult must provide their own permission.
2. Where and why the work may be used
Permission is limited to displaying Skitzo Productions’ work on its website, portfolio and official social media accounts, and in digital advertisements promoting its own photography and video services. The work may appear online worldwide. This does not authorize sale as stock imagery, merchandise, unrelated brand advertising or licensing my likeness to other businesses. Hosting, social media and advertising providers may process the work only as needed for these uses.
3. Editing and respectful use
Skitzo Productions may select, crop, resize, color-correct, retouch and edit the work for the permitted purposes. It may not use the work in a defamatory, deceptive, sexual or otherwise unlawful context, imply an unrelated endorsement, or create a synthetic likeness or voice without separate written consent. My full name and personal contact details will not be published without separate permission.
4. Payment and other agreements
I understand that I will not receive additional payment or royalties for these permitted uses unless agreed separately in writing. This release does not change session pricing, delivery terms, copyright ownership or any separate client agreement. Permission is voluntary; declining this release does not itself change the services purchased.
5. Duration and withdrawal
Permission begins when this form is submitted and continues until I withdraw it by emailing skitzoprod@icloud.com and identifying the session. Skitzo Productions will stop new promotional uses and remove the work from places it controls within 30 days of receiving the request. Withdrawal does not make earlier authorized uses unlawful. Copies, shares or caches controlled by others may remain online.
6. Who may sign and electronic consent
I am at least 18 and sign for myself, or I am at least 18 and the parent or legal guardian authorized to give this permission for the minor named below. I have read this release, understand its limited purposes and agree voluntarily. By entering my full legal name in the electronic signature field and submitting this form, I intend to sign this release electronically and consent to keeping it as an electronic record.
